About this service
Continua Health provides CareNav, a chronic care management platform used by healthcare providers ("Providers") to support patients enrolled in a care management program. As part of this service, patients may receive automated phone calls and text messages from our AI care coordinator, Riley, on behalf of their Provider. By participating in a check-in call or text conversation, you agree to these terms.
Calling and messaging consent
By providing your phone number to your Provider, or by responding to a check-in call or text, you consent to receive automated calls and text messages from Riley at that number as part of your care management program. Message frequency varies. Message and data rates may apply. You can reply STOP at any time to opt out of text check-ins, or HELP for assistance.
Not for medical emergencies
Riley is not designed or intended to detect, diagnose, treat, or respond to medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not rely on Riley or the CareNav platform for emergency care of any kind.
No medical advice
Riley does not provide medical advice, diagnosis, or treatment. Information shared during a check-in is passed along to your care team, who are responsible for reviewing it and determining any appropriate follow-up. Your Provider — not Continua Health — remains responsible for your clinical care.
Accuracy of check-ins
Check-in summaries are generated with the help of automated tools and are reviewed by a member of your care team before any action is taken. While we aim for accuracy, these summaries may not capture every detail of a conversation, and your care team's review is an important part of the process.
Limitation of liability
To the fullest extent permitted by law, Continua Health is not liable for indirect, incidental, or consequential damages arising from use of the platform. The platform is provided on an "as is" basis without warranties of any kind, express or implied.
Changes to these terms
We may update these terms from time to time. We'll post the updated version here with a new "last updated" date.
Governing law
These terms are governed by the laws of the State of California, without regard to its conflict of law principles.
