Overview

Continua Health provides a chronic care management (CCM) platform, CareNav, used by healthcare providers to support their patients between office visits. As part of this service, our AI care coordinator, Riley, conducts brief check-in phone calls and text (SMS) conversations with patients who are enrolled in a care management program with their healthcare provider. This policy explains what information we collect through those check-ins, how it's used, and the choices you have.

Information we collect

When you participate in a check-in call or text conversation, we may collect:

• Your name, phone number, and identifying information provided by your healthcare provider
• The content of your check-in conversation, including what you share about how you're feeling, medication adherence, recent ER or hospital visits, and your care plan goals
• Call and message metadata, such as the date, time, and duration of a check-in

How we use this information

Information gathered during a check-in is used to generate a clinical summary for your care team to review as part of your ongoing care management. We do not use your health information for advertising, and we do not sell your information to third parties.

SMS check-ins

If your provider enables text check-ins, you may receive automated messages from Riley asking about your health and care plan. Message frequency varies based on your care plan. Message and data rates may apply. Reply STOP at any time to stop receiving text check-ins, or HELP for assistance.

How we share information

Your check-in information is shared with your healthcare provider's care team, since that's the purpose of the service. We also work with third-party service providers — such as telephony, messaging, and language-processing vendors — who process information on our behalf solely to operate the platform, under confidentiality obligations. We do not sell your information, and we only disclose it beyond these purposes when required by law.

Security

We apply administrative, technical, and physical safeguards designed to protect your information, including encrypting sensitive health information at rest. No system is perfectly secure, but we work to continually improve our safeguards as our platform evolves.

Your choices

You can opt out of text check-ins at any time by replying STOP, or by contacting your healthcare provider directly. For questions about how your information is used, or to request access to or correction of your information, contact us using the details below or speak with your care team.

Changes to this policy

We may update this policy from time to time as our platform evolves. We'll post the updated version here with a new "last updated" date.
